Why Google Forms isn't a healthcare safety reporting system

For quality and patient safety teams running reporting on general-purpose tools

Google Forms is a quick way to start collecting safety concerns, but responses stop in a spreadsheet. This page explains what happens after submission, what Google's HIPAA business associate agreement covers and requires, and what a reporting system adds: routing, review, patterns and feedback.

Key takeaways

  • Google Forms is covered by Google's BAA for Workspace customers, but only after an administrator accepts it.
  • Personal Google accounts and third-party add-ons aren't covered.
  • A form collects reports; someone still has to route, track and reply to each one.
  • Consistent categories and severity levels are what make trends visible.

Where Google Forms fits

Google Forms is free, familiar and fast to deploy, and healthcare teams have used it to get a reporting mechanism in place quickly when nothing else exists. That pragmatism makes sense.

The challenge is what happens after submission. Responses land in a Google Sheet with no routing, review workflow or way to reply to the person who submitted. Someone has to read each response, decide who should see it and follow up by hand.

On HIPAA, Google Forms is among the services covered by Google's business associate agreement for Google Workspace customers, as part of Google Drive. An administrator has to accept the BAA in the Admin console before any protected health information is used, third-party add-ons aren't covered, and the BAA applies to Workspace accounts, not personal Google accounts. Safety events often include patient details, so confirm the account and BAA before using a form for them.

Structure matters too. A free-text form or a short list of checkboxes lets you count submissions, but it doesn't produce the consistent categories and severity levels you need to spot patterns.

When frontline staff submit a concern and never hear back, they stop submitting. A safety reporting program depends on closing that loop.

Questions to ask in any evaluation

Put the same questions to every option, including us, and get the answers in writing.

  • Is the form in a Google Workspace account with the BAA accepted, or a personal account?
  • Who reads each response, and how quickly?
  • How do reporters hear what happened?
  • Are categories and severity consistent enough to show trends?

ImprovementFlow

Safety Reporting

Safety Reporting picks up where a form stops: each report is routed to the right review team, tracked to action, and the reporter hears what was done. HIPAA compliance, a BAA and SSO are included in every package.

How ImprovementFlow compares

ImprovementFlow picks up where a form stops. Staff report from any device or a QR code on the unit, using your own event categories and severity levels, so reports arrive as structured data.

Each report is routed by event type, department and severity to the team that reviews it, and carries an owner, a status and tags. Reviewers look across events to see what keeps recurring, and named reporters receive feedback on what was done.

HIPAA compliance, a BAA and SSO are included in every package.

Bringing your records across

If you've been collecting responses in Google Sheets, export them as CSV and bring your question list. We'll map your categories with you and import your past responses, so your history comes with you.

What ImprovementFlow includes

  • Routing by event type, department and severity to the team that reviews it
  • An owner, status, severity and tags on every event, with feedback to named reporters
  • Patterns across departments, severities and tags

Frequently asked questions

How is ImprovementFlow priced?
Pricing is by program bundle, not per user, and HIPAA compliance, a BAA and SAML/SSO are included in every package. We don't publish prices on comparison pages. See the /pilot page for details.
How does moving from Google Forms work?
We start from the event categories, severity levels and review structure you already use, and set up categories and routing rules with you. Implementation support and training are included in every package. In a 90-day pilot, setup takes the first two weeks and staff start reporting on real data in week 3. We import your past records from your current system, spreadsheets or paper archives, so your trend history comes with you. After setup, your team changes event types, forms, categories and routing itself, without IT.
Can we pilot ImprovementFlow before fully switching from Google Forms?
Yes. The 90-day pilot runs ImprovementFlow on a single program at a defined scope, with success criteria agreed at kickoff. See the /pilot page for details.
