Key takeaways
- Since October 2019, IRFs capture function in Section GG of the IRF-PAI rather than the FIM instrument.
- Rehab teams get data they trust when they define their own measures instead of borrowing acute care ones.
- Common measures include discharge to community, change in self-care and mobility, and unplanned transfers to acute care.
- Give every measure an owner and link off-target measures to an improvement project.
Rehab measures in acute care tools
Inpatient rehabilitation facilities (IRFs) measure different things from acute care hospitals: functional outcomes in self-care and mobility, therapy-specific events and discharge destination. Since October 2019, function has been captured in Section GG of the IRF Patient Assessment Instrument, which replaced the FIM instrument. When an IRF borrows its parent hospital's quality platform, those measures don't fit, and staff work around the tool.
The CMS IRF Quality Reporting Program has its own measures and assessment data requirements. Managing them in a system built for acute care means constant translation between the two.
Freestanding rehab facilities have no parent system to borrow from. Many manage quality in spreadsheets and paper, which makes it hard to show surveyors and accreditors a consistent record.
Building quality work around rehab outcomes
Quality work goes faster when the system matches the clinical work. Rehab teams that define their own measures, such as functional improvement, therapy-related events, length of stay and discharge destination, get data they trust and use.
IRF-PAI assessments still go to CMS from your clinical software. What a quality program adds is ownership: a named person watching each measure between CMS reports and an improvement project behind every one that slips.
ImprovementFlow
Quality Reporting
Quality Reporting lets rehab teams set up the measures they manage themselves, with a named owner for each, and link an off-target measure to the improvement project working on it.
Measures rehab teams commonly own
- Discharge to community, by diagnosis group.
- Change in self-care and mobility scores from admission to discharge.
- Falls during therapy and on the unit.
- Unplanned transfers back to acute care.
- Therapy time delivered compared with the plan of care.
Getting set up
Whether you're a freestanding IRF moving from spreadsheets or a hospital-based unit leaving the parent hospital's system, we set up your measures and event categories with you and import your historical data.
How ImprovementFlow supports this
- Rehab teams set up the measures they manage themselves, without waiting on an analyst.
- Each measure has a named owner and a review cadence, from the daily huddle board to the quality committee.
- An off-target measure can be linked to the improvement project addressing it.
- HIPAA compliance, a BAA and SSO are included in every package.
