Key takeaways
- Report abuse or serious bodily injury within 2 hours and other covered events within 24 hours; investigation results are due in 5 working days.
- QAPI requires at least one performance improvement project a year on a high-risk or problem-prone area.
- SNF VBP now uses eight measures, including falls with major injury, staffing and staff turnover.
- The 2024 federal staffing minimums were repealed in February 2026; the 8-hour daily RN requirement remains.
What makes safety reporting hard in long-term care
Nursing homes work under one of the most demanding regulatory regimes in healthcare. Federal requirements, state licensure and mandatory incident reporting all demand documentation, trending and evidence of corrective action, while a single nursing assistant may be caring for many residents on a shift and every documentation task competes with hands-on care.
Reporting deadlines are measured in hours. Allegations of abuse, neglect and injuries of unknown source must be reported within 2 or 24 hours. When an event happens at 2 a.m., documentation has to start immediately, even if the investigation can't finish until morning. A system that needs a desktop and an administrator login doesn't fit that reality.
Survey readiness means showing a pattern of response, not just a pile of reports. Surveyors look for root causes identified, corrective actions taken and those actions monitored. Assembling that from incident reports, care plans and email takes administrators away from operations at the most stressful time of the year.
The nursing home regulatory landscape
Every nursing home, including those in multi-facility chains, must run a comprehensive, data-driven QAPI program under 42 CFR 483.75. It must monitor adverse events, use root cause analysis and systemic action, and conduct performance improvement projects, including at least one a year on a high-risk or problem-prone area.
- Abuse, neglect and injuries of unknown source (42 CFR 483.12(c)): report immediately, and no later than 2 hours if the event involves abuse or serious bodily injury, otherwise within 24 hours, to the administrator and the State Survey Agency. Investigation results are due within 5 working days.
- State surveys: each facility gets a standard survey no later than 15 months after the last, with a statewide average of 12 months.
- Care Compare's Five-Star Quality Rating combines health inspections, staffing and quality measures. Since July 2025, the inspection rating uses the two most recent standard surveys.
- Skilled Nursing Facility Value-Based Purchasing has grown from a single readmission measure to eight measures for FY2027, including falls with major injury, staffing, staff turnover and healthcare-associated infections.
- The 2024 federal minimum staffing standards were repealed effective February 2, 2026. The statutory requirements for 8 hours of RN coverage a day and a full-time director of nursing remain, as does the facility assessment.

Safety Reporting
With 2-hour and 24-hour reporting clocks, a report has to start at the point of care at any hour. Safety Reporting takes reports from any device, routes them by type and severity, and keeps the owner, status and linked follow-up together for survey.
What to prioritize first
- 1Make sure any nursing assistant or nurse can start a report at the point of care, at any hour, so the 2-hour and 24-hour clocks are never missed.
- 2Set up categories for the events that matter in long-term care: falls with and without injury, medication errors, elopement, abuse and neglect allegations, pressure injuries and behavioral incidents.
- 3Keep each event's investigation, corrective action and follow-up monitoring together, so survey evidence builds as you go.
- 4Choose this year's performance improvement project from your own event data, such as falls with major injury.
- 5Watch staff well-being and turnover; both affect resident safety and now count in SNF VBP.
How ImprovementFlow supports this
- Reporting from any device at the point of care, routed by event type and severity to the right reviewer immediately.
- Configurable event categories for long-term care, each with an owner, a status, reviewers and a linked improvement project when needed, so the record from report to follow-up stays in one place.
- Well-being assessments and pulse checks that show which units and roles are under the most strain.
